Formulary mappings
Pair pharmacy medications with your protocols and set the directions, strength, size, and days supply your providers start from.
This is where a Provider Network Admin pairs medications from your connected pharmacies with the protocols in your catalog, and sets the default directions a provider sees when they prescribe.
This page only exists once a pharmacy is connected. The Formulary item appears in the left menu only when your network has at least one enabled pharmacy whose source is OnlyScripts. With none, the nav item and the /formulary route are both removed — typing the URL gives you Page not found — The page you're looking for doesn't exist or you don't have access. Finish Integrations and pharmacy first.
Questions people ask about this page
I connected OnlyScripts and Formulary still is not in the menu.
The answer to "does this network have an OnlyScripts pharmacy" is cached in your browser for 24 hours and revalidated in the background. Reload the page. If it still does not appear, confirm on /integrations that a pharmacy row under Pharmacies is switched on — an integration that is present but not enabled does not count.
Do I set the patient price here?
No. The Price, Shipping, and Total columns show what the pharmacy charges — they are read-only reference values from the fulfilment path. What a patient pays is set by the client on its own Products page.
What is the difference between pairing here and pairing from Protocols?
Nothing — they write the same pairing from opposite ends. On /formulary you start from a medication and click its Protocols cell to pick catalog items. On /protocols you start from a product and use Manage formulary defaults to pick medications. Use whichever direction matches the job.
Does a pairing decide what the provider prescribes?
No. A pairing is a reviewed starting point. The prescribing provider still sees and can change every field in the charting modal's prescribe step before anything is sent.
A medication I need is not listed.
The table only ever contains what your enabled pharmacies return. Check that the right pharmacy is on, that any pharmacy-specific credentials are filled, and that the medication is not hidden by an active filter. If it genuinely is not carried, add it with Add custom formulary — but a custom medication is save-only and cannot be transmitted to that pharmacy.
What is the Days supply field for?
It is the default number of days one dispense covers, used when the prescription is written. Leave it blank and the value falls through to the product, then to the standardised, calculated, or platform default. Set it only where the network has agreed a number.
Can a Provider see this page?
No. Formulary is Provider Network Admin only. Providers and Provider Delegates have no nav item and the page falls through to the message This page is only available to formulary-enabled tenant administrators and staff.

Left menu → Formulary. Direct link: /formulary. Visible to: Provider Network Admin only, and only when the network has an enabled OnlyScripts pharmacy. Page heading Formulary, subtitle Pair protocols with OnlyScripts medications from enabled pharmacies. There are no tabs.

How to pair a medication with a protocol
Open the page
Click Formulary in the left menu, or go to /formulary.
Narrow to one pharmacy
Click the pill for the pharmacy you intend to dispense from. Two pharmacies often carry the same molecule at different strengths and prices, so pairing the wrong row is the usual mistake.
Find the exact formulation
Type into Search medications or SKU.... Confirm the Strength, Form, and Size columns match what your clinical team approved before you touch anything.
Open the pairing sheet
Click the Protocols cell on that medication's row. A sheet opens headed with the medication name: Choose catalog products paired with {medication} from {pharmacy}. Changes sync with the formulary dashboard and prescribing flow.
Pick the catalog items
Search with Search products... and tick each protocol or product this medication should back. Cycle the status pill if you want to see only paired or only unpaired items. If nothing is listed you get No catalog products loaded for formulary pairing.
Save
Save the sheet. A failure shows Unable to save formulary pairings. or Unable to save formulary mapping. — retry rather than creating a second pairing.
Confirm on the row
The Protocols cell now lists the items you picked. On /protocols, the matching row's Formulary cell counts up.
How to set directions and days supply
Open the medication defaults sheet
Click the row's edit control to open the per-medication defaults sheet. Arrow between medications with Previous medication and Next medication without closing it.
Write the directions
Directions is the patient-facing sig. Placeholder e.g. Take 1 tablet by mouth once daily. Only enter directions the network has approved as a standard — a provider can override them, but most will not.
Confirm the product facts
Strength (e.g. 10 mg), Form (e.g. Tablet), and Size (e.g. 30 count) describe what is dispensed.
Set days supply
Days supply (e.g. 30) must be a positive whole number. Blank means "fall through to the next default", which is usually what you want unless the network has agreed otherwise.
Add the optional statements
Clinical difference and Medical necessity (both Optional) are free text some pharmacies require on compounded orders. Leave them empty unless your pharmacy asks for them.
Save before you leave
Closing with unsaved edits prompts Discard unsaved changes? A partial save across several pharmacies reports Unable to save medication defaults for some pharmacies. — reopen and check each one.
How to add a medication your pharmacy does not carry
Open the sheet
Click Add custom formulary. The sheet is titled Custom formulary — Add or edit non-OnlyScripts medications for save-only prescribing. With none yet it reads No custom medications yet.
Fill the record
Medication name, Strength (e.g. 10 mg/mL), Dose form (e.g. cream), Directions (Patient-facing sig), Days supply (Optional), and an Active toggle.
Know the limitation
"Save-only" is literal: a custom medication is recorded on the chart but is never transmitted to a pharmacy. Use it for medications a patient fills elsewhere, not as a workaround for a missing pharmacy item.

Price, Shipping, and Total are not settings. They come from the pharmacy and cannot be edited here.
